Services Provided

  1. Periodic Assessments of Your Loved One.
    A ForeverCare Associates representative will visit with your loved one.  During the visit, questions will be asked in a non-intrusive manner to assess if any abuse, neglect, or exploitation is occuring or has a potential to occur.

  2. Review the Medical File of Your Loved One.
    To assess if any warning signs of abuse or neglect exist, a ForeverCare Associates representative will conduct a thorough examination of your loved ones file (if they reside at the nursing home or assisted living facilty).  We will also assess if the best possible care, treatment, and equipment (when applicable) is being provided for them. 

  3. Assessment of the Staff and the Facility.
    While at the nursing home, assisted living facility, or if assisted home care is being provided, the ForeverCare Associates representative will conduct a general overview of the attitudes of the care providers, interaction with residents, cleanliness of the home or facility, safety and security of the home or facility, and overall atmosphere. 

  4. Written Assessments
    You will be provided a written summary of the findings resulting from the assessment of your loved one.  Included in the written assessment will be recommendations to address any issues.

  5. Issue Resolution
    ForeverCare Associates  will develop a plan of action to address every issue discovered during their assessment and assist you in implementing the plan upon request.


    Additional Services Available:

    In addition to the services listed above, ForeverCare Associates (FCA) has been retained to assist in a variety of special circumstances such as:

    Developing a system to eliminate a family member from constantly harassing an elderly family member for financial support.  (FCA retained by daughter)

    Conducting periodic assessments of a mentally disabled adult residing outside the home of the parent. (FCA retained by a terminally ill father)

    Developing a monitoring system to assist an elderly client in her financial matters while still maintaining her independence. (FCA retained by daughter)

    Conducting periodic assessments of an elderly individual that has caregivers providing care to them in their home. (FCA retained by niece)

    Future assessments of an individual when/if she becomes incapable of taking care of herself. (Hired in advance by a senior citizen for FCA to monitor her once she moves to a nursing home or has a live-in caretaker).
